A medical supplies delivery note: batch and expiry for every item.

A clinic or lab receiving supplies wants the batch number and expiry from the moment of delivery, because that is what it turns to for any query. This template puts batch and expiry in two columns per item, and leaves a received column for the count. It has sample rows with no brands and no prices.

Common questions

What if expiry is close on arrival?

Note it on the delivery note before signing, and decide with the supplier whether to accept or refuse it. A date written at the moment of receipt protects you in a dispute.

Should I check packaging is intact?

Yes, sterile supplies cannot be accepted in a torn pack. Check the seals and record any damaged pack, entering a lower figure in the received column.

How many copies should I keep?

At least one for you and one for the supplier. For items that need traceability, keep the note in the supplier's file.

Dashing does this for every item, automatically, in your shop.
